By the time I started watching Doctor Who regularly I was already an adult (as adult as I will ever be, I'm guessing), so I'm not sure I've ever found a villain particularly scary. Though there is something about the Daleks and Cybermen that is always frightening to me: you can't reason with them. Just like any sci-fi baddy that is predominantly metal/mechanical/computer/robot in nature. And with the Daleks and Cybermen, there's no logical paradox to make them self-destruct. They just want to destroy and consume.
But I think regarding villains that are just straight-up creepy I'd have to say the Dream Lord takes the cake. Talk about a fantastic villain. And when you find out where he's really from ... that makes him all the more compelling and absolutely fascinating as an antagonist to the Doctor.
